SUPPORTED LIVING: 1 to 3 residents live together and control their own house or apartment with up to 24 hour support from Hilltoppers, Inc. staff.
SEMI INDEPENDENT LIVING: 1 to 2 residents live together and control their own house or apartment with limited support of Hilltoppers, Inc. staff.
RESIDENTIAL HABILITATION: 3 to 10 service recipients living in one congregate setting with 24 hour support of Hilltoppers, Inc. staff.
MEDICAL RESIDENTIAL: 1 to 4 residents live together with a Nurse on duty to provide skilled nursing.
FAMILY MODEL: 1 to 2 service recipients live with a family who are paid to support these residents with up to 24 hour support.
PERSONAL ASSISTANCE: One service recipient with one staff person who supports the person in his or her needs, at the person's home for a specific number of hours per week.
RESPITE: Emergency short-term care for service recipients needing temporary residential services due to a family emergency.
TRANSPORTATION: Hilltoppers, Inc. provides transportation to and from the person's home to the day center, community participation events, medical appointments and other necessary places.
