CHOICES:

CHOICES includes assistance with all daily living skills, medication management & administration and other services according to the person's level of need.

Adult Day -  day care away from the person's home Monday through Friday.

Personal Care - assistance with toileting, dressing, hygeine, bathing, mobility, etc. at the person's home.

Homemaker -  sweeping, mopping, dusting, bed making, washing dishes, laundry, meal preparation Etc, at the person's home.

In Home Respite - companionship for an extended period of time when a family member or care giver needs a break. Services can include personal care and homemaker services.

CLS - Community Living Supports - 24 hour care in a residential setting.

ECF (Employment and Community First):

Community Integration Skills Supports - community supports to learn integration skills and build relationships.

Independent Living Skills - to learn specific life skills to live more independently at home.

Discovery - assessement to learn about the person's job interests and workability.

Exploration - exposure in the community to define what the person desires to do with their life.

Employment - supports to assist the person in seeking, finding, and keeping a job.

CLS - Community Living Supports - 16 hours per week up to 24 hours per day supports in a residential setting.
